At Michael J. Ilardi, DMD , we offer comprehensive general dentistry services to ensure that you look and feel your best. We understand that going to a new doctor’s office can be a bit confusing, but rest assured we will explain what you can expect from us every step of the way. To allow us to serve you better on your first appointment, be sure to bring:
- Identification
- Insurance cards or forms provided to you by your insurance company
- A list of any current medications you may be taking
Appointments
In case you may not be able to make your appointment, please provide us with 24 hours notice so we can schedule this appointment for another patient. If a patient doesn’t show up for an appointment without notice, a fee may be charged.
Office Policies
Our office is here to provide you great service, but we are also here to keep you and your information safe. We are a HIPAA Compliant office, and rely on your consent and agreement to HIPAA Service, and insurance (when applicable) Policies to provide you care.
Payments
Payments are due at the time of service. For your convenience, we are happy to accept Cash, Checks, Visa, MasterCard and American Express and will file all insurance on your behalf. Alternative payment methods are available upon inquiry.
